Cholecystitis on MRI: What the Examination Shows and When It Is Recommended

Cholecystitis is inflammation of the gallbladder that may be accompanied by pain in the right upper abdomen, nausea, vomiting, fever, and impaired bile flow. In most cases, abdominal ultrasound is the first-line diagnostic examination. A doctor may recommend magnetic resonance imaging when a more detailed assessment of the gallbladder, bile ducts, and surrounding tissues is required or when previous examination results are inconclusive.

In brief: MRI can show signs of gallbladder inflammation, changes in the gallbladder wall, fluid around the organ, gallstones, dilated bile ducts, and possible complications. MRCP—magnetic resonance cholangiopancreatography—is often used for a detailed examination of the biliary system.

What Is Cholecystitis?

The gallbladder is a small organ located beneath the liver. It stores bile, which helps digest fats, and releases it into the duodenum after a meal.

Cholecystitis develops when the gallbladder wall becomes inflamed. The most common cause is impaired bile drainage due to a gallstone blocking the cystic duct. However, inflammation can also occur without gallstones. This condition is known as acalculous cholecystitis.

Main Types of Cholecystitis

  • Acute cholecystitis — inflammation develops suddenly and is usually accompanied by pronounced symptoms.
  • Chronic cholecystitis — a long-term or recurrent inflammatory process that may gradually change the gallbladder wall.
  • Calculous cholecystitis — inflammation associated with gallstones.
  • Acalculous cholecystitis — inflammation without detectable gallstones, more commonly associated with serious illness or impaired blood supply to the gallbladder.

What Symptoms Can Occur with Cholecystitis?

Symptoms depend on the form of the condition, the severity of inflammation, and whether bile flow is obstructed. Possible symptoms include:

  • pain in the right upper abdomen or upper central abdomen;
  • pain that becomes worse after eating fatty or fried foods;
  • pain radiating to the right shoulder blade, shoulder, or back;
  • nausea and vomiting;
  • a bitter taste in the mouth;
  • bloating and a feeling of heaviness after eating;
  • fever;
  • loss of appetite;
  • yellowing of the skin or whites of the eyes when bile flow is obstructed.

Similar symptoms may occur with pancreatitis, peptic ulcer disease, and disorders of the liver, bile ducts, kidneys, or other organs. For this reason, a diagnosis cannot be made based on the nature of the pain alone.

When Is Urgent Medical Attention Required?

Do not wait for a routine examination if pain in the right upper abdomen:

  • is severe and does not improve for several hours;
  • is accompanied by a high temperature or chills;
  • occurs together with repeated vomiting;
  • is accompanied by jaundice;
  • occurs together with severe weakness, dizziness, or loss of consciousness;
  • gradually becomes worse and spreads throughout the abdomen;
  • is accompanied by pronounced abdominal muscle tension.

These symptoms may indicate acute inflammation, obstruction of the bile ducts, or the development of complications and require urgent medical assessment.

Is MRI the Main Diagnostic Method for Cholecystitis?

MRI is usually not the first examination performed when acute cholecystitis is suspected. A doctor will generally begin with an abdominal ultrasound because it can quickly assess the gallbladder, identify most gallstones, and detect changes in the gallbladder wall and other signs of inflammation.

MRI or MRCP may be recommended as the next diagnostic step when:

  • the ultrasound findings are inconclusive;
  • symptoms persist but their cause has not been identified;
  • gallstones in the common bile duct are suspected;
  • blood tests show elevated bilirubin or liver enzyme levels;
  • dilated bile ducts have been detected;
  • possible complications need to be assessed;
  • the gallbladder, liver, bile ducts, and pancreas need to be examined at the same time;
  • an examination without ionising radiation is preferred;
  • there are contraindications to certain types of CT contrast agents.

What Does Cholecystitis Look Like on MRI?

During an MRI examination, the radiologist evaluates the size and shape of the gallbladder, the condition of its wall, its contents, the surrounding tissues, the liver, the bile ducts, and other organs in the upper abdomen.

In cases of cholecystitis, MRI may show:

  • enlargement or deformation of the gallbladder;
  • thickening of the gallbladder wall;
  • swelling of the gallbladder wall;
  • fluid collecting around the gallbladder;
  • inflammatory changes in the surrounding tissues;
  • a gallstone in the gallbladder or bile duct;
  • impaired bile drainage;
  • dilation of the intrahepatic or extrahepatic bile ducts;
  • changes in the adjacent areas of the liver;
  • possible complications of the inflammatory process;
  • associated changes in the pancreas and pancreatic duct.

A single MRI finding does not always confirm cholecystitis. For example, thickening of the gallbladder wall can occur in other conditions. The doctor therefore evaluates the combination of imaging findings together with the patient’s symptoms, blood test results, and ultrasound findings.

What Does Acute Cholecystitis Look Like on MRI?

In acute inflammation, MRI images may show an enlarged gallbladder, thickening and swelling of its wall, fluid around the organ, inflammatory changes in the surrounding tissues, and a gallstone obstructing bile flow.

If the inflammatory process spreads, MRI may also help assess the surrounding fatty tissues, liver, and bile ducts. However, in an emergency, the most appropriate diagnostic method is selected by the doctor according to the patient’s condition and the availability of imaging examinations.

What Can MRI Show in Chronic Cholecystitis?

In chronic cholecystitis, the gallbladder may be deformed or reduced in size, and its wall may show irregular changes. MRI may also reveal gallstones, thick bile, abnormalities in the anatomy of the biliary system, and associated changes in the ducts.

The signs of chronic cholecystitis are not always specific. The final clinical assessment is based on the patient’s medical history, recurrent episodes of pain, ultrasound findings, laboratory test results, and consultation with a gastroenterologist or surgeon.

What Is MRCP?

MRCP stands for magnetic resonance cholangiopancreatography. It is a specialised MRI protocol used to examine the bile ducts and pancreatic ducts.

MRCP provides detailed images of:

  • the gallbladder;
  • the cystic duct;
  • the common bile duct;
  • the intrahepatic bile ducts;
  • the main pancreatic duct;
  • the area where the ducts open into the duodenum.

This protocol is particularly useful when a doctor suspects a stone in the bile duct, narrowing or dilation of a duct, or another cause of impaired bile flow.

How Does MRCP Differ from a Standard Abdominal MRI?

When booking the examination, it is important to provide the exact wording from the medical referral. A standard abdominal MRI and MRCP have different diagnostic purposes and cannot always replace one another.

Can Gallstones Be Seen on MRI?

MRI and MRCP can detect gallstones in the gallbladder and bile ducts. However, ultrasound is often sufficient for the initial detection of stones located within the gallbladder.

MRCP is particularly valuable when a gallstone is suspected to have moved from the gallbladder into the common bile duct. This may cause obstruction, jaundice, inflammation of the bile ducts, or pancreatitis.

There is no single best imaging method for every patient. The doctor selects the examination based on the symptoms, blood test results, urgency, and previous diagnostic findings.

Is Contrast Needed When Cholecystitis Is Suspected?

MRCP of the bile ducts is often performed without intravenous contrast. Special MRI sequences make it possible to visualise fluid within the bile ducts and pancreatic ducts.

Contrast-enhanced abdominal MRI may be recommended when it is necessary to:

  • assess the activity of the inflammatory process in greater detail;
  • investigate changes in the liver or pancreas;
  • distinguish inflammation from a tumour;
  • assess the blood supply to tissues;
  • clarify the nature of complications;
  • examine a detected mass or lesion in greater detail.

The decision to use contrast is made by the doctor according to the diagnostic objective. A prescribed examination should not be replaced with a different protocol without medical advice.

How to Prepare for Gallbladder MRI and MRCP

The exact preparation requirements depend on the examination protocol. When booking, the administrator will explain how long before the procedure you should avoid food and whether drinking water is permitted.

General recommendations include:

  • bring your medical referral, if available;
  • bring previous ultrasound, CT, MRI, and laboratory test results;
  • confirm how many hours before the examination you should stop eating;
  • avoid heavy and fatty foods before the examination according to the centre’s instructions;
  • inform the staff in advance if you are pregnant;
  • report any pacemaker, implants, prostheses, vascular clips, or other metal or electronic devices;
  • remove jewellery, watches, piercings, and other metal objects;
  • inform the staff if you have claustrophobia;
  • do not stop taking regular medication without medical advice.

If contrast enhancement is planned, inform the staff about kidney disease, pregnancy, breastfeeding, and any previous reactions to contrast agents. An up-to-date blood test for creatinine may be required.

How Is an MRI of the Biliary System Performed?

Before the procedure, the patient completes an MRI safety questionnaire and leaves metal objects outside the examination room. During the scan, the patient must lie still on the scanner table.

To obtain clear images of the abdominal organs, the radiographer may ask the patient to hold their breath for several seconds. The scanner produces loud, rhythmic sounds, which are a normal part of the examination.

After the scan, the radiologist analyses the images and prepares a report. The results should then be reviewed by a gastroenterologist, surgeon, or the doctor who referred the patient for diagnostic imaging.

What Should You Do After Receiving the Results?

An MRI report is not a final clinical diagnosis and does not by itself determine the treatment method. The doctor compares the imaging results with the patient’s symptoms, physical examination, laboratory findings, and ultrasound results.

Depending on the findings, the following may be required:

  • consultation with a gastroenterologist;
  • consultation with a surgeon;
  • additional blood tests;
  • a follow-up ultrasound;
  • an endoscopic examination of the bile ducts;
  • planned or urgent treatment.

Хочу подякувати адміністратору центра..і тут починається саме цікаве.Як часто ,Ви, як пацієнти,запам'ятовуєте ім'я адміністратора,який(, чи яка) є зв'язковим між нами, пацієнтами, та лікарями???Ось і я потрапила у таку незручну ситуацію. Але,я гадаю,що Пані згадає пацієнтку,яка годину не давала спокою.Це було 16.10. Ви були дуже чуйною,терпляче мене вислуховували,заспокоївали,вирішували .
