Signing a declaration with a family doctor at the HBMedical medical center is an official confirmation that you have chosen our institution to receive free medical services under the National Health Insurance Program.
This declaration guarantees you constant medical support, access to high-quality primary care, and the ability to comprehensively monitor your health.
What is needed to sign a declaration at HBMedical
To complete a declaration with a family doctor, you must have:1
- Passport;
- Identification code (RNOKPP);
- Your mobile phone number.
The procedure takes only a few minutes, after which you immediately become a patient of our medical center.

Medical services provided after signing the declaration
After signing the declaration, you have the right to receive free of charge:
- Consultation of a family doctor or therapist;
- Referral for a complete blood count with leukocyte formula;
- Referral for a complete urine test;
- Referral for determination of blood glucose and total cholesterol levels;
- Measurement of blood pressure, weight, height, waist circumference;
- Preventive examinations and referrals to specialists as needed.
- Electrocardiogram (ECG);

Program “Affordable Medicines”
Patients who have signed a declaration with a family doctor HBMedical can receive prescriptions for drugs under the government program “Affordable Medicines”.
Prescriptions are written out during a consultation with a family doctor, therapist or pediatrician, according to medical indications.
Declaration validity period
Medicines can be obtained free of charge or with a partial co-payment in pharmacies marked with the “Affordable Medicines” logo.
The declaration is signed indefinitely. If you wish to change your doctor, simply contact your chosen specialist with the necessary documents. After confirming the change in the eHealth system, the declaration is updated automatically.
