What is metal osteosynthesis
Metal osteosynthesis (MOS) is a modern surgical method of treating fractures, which involves stable fixation of bone fragments using metal structures. Depending on the clinical situation, plates, screws, intramedullary rods or external fixators are used.
The main goal of MOS:
- restoration of the anatomical integrity of the bone;
- ensuring stability in the fracture zone;
- creating optimal conditions for union;
- restoration of limb function in the shortest possible time.
The method is especially important in complex and unstable fractures, open injuries, as well as in cases where conservative treatment is ineffective.
When metal osteosynthesis is indicated:
- Complex multi-fracture fractures of long bones
- Intra-articular injuries
- Open fractures with a risk of displacement
- Unstable fractures that do not heal on their own
- Pseudoarthrosis or nonunion of the bone
- Correction of bone deformities
- Reoperation after ineffective treatment

Types of osteosynthesis
- Internal osteosynthesis – plates, screws, intramedullary rods are used
- External osteosynthesis – Ilizarov apparatus or other modular systems
- Combined osteosynthesis – a combination of internal and external techniques
- Minimally invasive MOS (MIPO) – reduces tissue trauma, promotes faster recovery
How is the MOS operation performed
- Anesthesia. Depending on the location and complexity of the fracture, spinal or general anesthesia is used, which provides comfort and painlessness for the patient.
- Surgical access. The surgeon makes a small incision in the fracture area, preserving soft tissues and neurovascular structures as much as possible.
- Reposition of fragments. The bone fragments are precisely aligned with each other, the anatomical shape of the bone and articular surface is restored (if the damage is intra-articular).
- Fixation. Special metal structures are used for stabilization – plates with screws, intramedullary rods or external devices. The choice depends on the type of fracture and clinical situation.
- Intraoperative control. If necessary, real-time X-ray control is used to ensure the correct position of the fragments and implants.
- Completion of the operation. The wound is sutured in layers, drainage is installed (if necessary), a sterile bandage is applied. In some cases, the limb is additionally fixed with an orthosis.
- Early postoperative period. After 1–2 days, the patient begins gradual movements and dosed load under the supervision of a doctor.
Postoperative period
- Hospitalization usually lasts 1–4 days
- Painkillers and antibiotic prophylaxis are prescribed
- Rehabilitation begins from the first days: exercise therapy, physiotherapy
- Stage control X-rays are performed
- The terms of consolidation (fusion) of the bone are usually 6–12 weeks
- In some cases, the structures are removed after complete fusion

Do I always need to remove metal plates after MOS?
No, if the structure does not interfere and does not cause inflammation, it can be left.
When can you put weight on the limb?
It depends on the type of fracture — sometimes from the first days, sometimes after 2–4 weeks.
What are the possible complications?
Infections, nonunion, migration, or fracture of the implant are rare, usually in complex cases or if recommendations are not followed.
Is it safe to have an MRI after a MOS?
Most modern implants are made of MRI-compatible titanium. Always inform your doctor about the presence of implants.
Are there any restrictions after surgery?
So, in the first period, you need to limit the load, wear an orthosis or brace, and follow the recommendations of a rehabilitation specialist.
