What is revision knee arthroplasty?
Revision knee arthroplasty is a surgical procedure to replace part or all of the components of a knee joint prosthesis. The goal of the surgery is not only to replace the implant, but also to restore knee stability, reduce pain, and restore the patient’s ability to move confidently.
Replacement of the knee joint endoprosthesis is recommended in cases of:
- Aseptic or septic loosening of the prosthesis components
- Infections of the knee joint area
- Wearing of the endoprosthesis components
- Periprosthetic fracture
- Dislocation, instability or deformation of the components
Methods of revision knee endoprosthesis
The choice of technique is determined individually, depending on the patient’s condition and the degree of damage:
- Replacement of one of the components
- Complete replacement of the endoprosthesis components
- Metal-osteosynthesis using plates
- Use of bone grafts to replace defects bone tissue
- Two-stage revision in infectious processes (first removal of endoprosthesis components, sanitation and installation of a temporary cement spacer, then installation of a new endoprosthesis)

Diagnosis and Preparation for Surgery
The effectiveness of revision knee arthroplasty depends largely on a comprehensive preoperative evaluation. The mandatory stages include:
- Consultation with an orthopedic traumatologist to determine the indications for re-intervention
- Imaging diagnostics: radiography in standard projections, if necessary – CT or MRI to clarify the condition of bone tissue and surrounding structures
- Laboratory tests, including blood tests and determination of inflammatory reaction markers
- Verification of possible foci of infection (if suspected)
- Individual selection of a revision endoprosthesis and/or the necessary components for metal osteosynthesis (MOS)
Performance of surgical intervention
Revision endoprosthesis is performed under general or spinal anesthesia and lasts an average of 2–4 hours. The main stages of the operation include:
- Removal of worn or infected endoprosthesis components
- Mechanical and antiseptic preparation of the bone and surrounding tissues
- Implantation of a revision knee endoprosthesis
- Application of metal structures or bone implants for additional stabilization in case of bone deficiency or periprosthetic fractures
- Completion of the surgical intervention and transfer of the patient to the ward
Rehabilitation after revision arthroplasty
The recovery phase is an integral part of successful treatment. From 1–2 days after the intervention, the patient is supported by a multidisciplinary team of rehabilitation specialists and doctors:
- Pain control and prevention of thromboembolic complications
- Early restoration of the range of motion in the knee joint using special exercises
- Learning to walk using additional resistance (crutches, cane)
- The time to achieve functional recovery is usually 3–6 months
- Regular monitoring of the condition of the implant and the reliability of its fixation, especially when using metal osteosynthesis
